Hardwood mulch and decorative rock solve different problems. Mulch breaks down over time, which is actually a benefit around plantings since it feeds the soil as it decomposes, but it needs refreshing every year or two to keep its color and depth. Rock doesn't break down and holds its look for years with almost no maintenance, but it doesn't do anything for the soil underneath and can hold more heat against plants in summer. Neither is universally better, it depends on the bed.
A weed barrier fabric makes sense under rock beds where you want long-term, low-maintenance coverage. Under mulch beds we're more selective about it, since it can interfere with the mulch's ability to feed the soil and tends to work its way to the surface over a few seasons anyway.
Usually every year to two years to keep the color and depth, since it naturally breaks down. Rock beds hold up much longer without needing a refresh.
Depends on the bed. Mulch feeds the soil as it breaks down but needs periodic refreshing. Rock is low-maintenance and permanent-looking but does nothing for the soil. We'll recommend based on what's actually planted there.
For a proper refresh, yes, built-up old mulch gets removed or thinned before new mulch goes down, otherwise you're just adding depth on top of decomposing material.
